For New Patients
Thank You for Choosing Our Practice
Every patient is unique, and we are committed to providing you with high quality, state-of-the-art periodontal and implant care based on your individual needs. Above all else, we want to make your visit to our office a pleasant experience.
What to Bring on Your First Visit
If you received or downloaded our welcome packet prior to your first visit to our office, please bring the completed forms with you to your appointment. Otherwise, please arrive a few minutes early to allow time to complete the medical/dental health questionnaire and the patient information form. Be sure to list all medications with dosages and any natural supplements, attaching an extra sheet of paper if necessary.
Also, bring your insurance cards for us to copy. If your general dentist has given you a written referral or x-rays, please remember to bring them with you to your appointment.
What to Expect on Your First Visit
Your first visit will take 30 minutes to 1 hour and will include the following:
- Review of your complete dental and medical history
- Comprehensive examination of your gums and teeth
- Review of your dental x-rays and/or taking new x-rays as required
- Cancer screening
- Occlusion and bite check
After a thorough exam, Dr. Carazola will discuss his findings and answer any questions you may have about treatment. His goal is to help you come to an informed decision regarding care.
Please remember to bring your dental insurance card with you on your first visit. We will file all the necessary paperwork to your insurance company, so you can receive the maximum reimbursement possible.
You are ultimately responsible for payment of all services. Should your insurance not cover what is predicted, you will be responsible to pay any uncovered portion.
Our knowledgeable staff will work with you to ensure any portion of treatment not covered by insurance is affordable. We accept cash, check, major credit cards (Visa, Master Card and Discover) as well as outside financing through Care Credit. The guidelines for acceptance by Care Credit are strictly followed. You can visit their website for more detailed information.
